Job Summary
Nurse Practitioner to provide comprehensive in-home and virtual health assessments for adult patients. You will conduct detailed clinical evaluations, interpret diagnostic data, and develop individualized care plans that support health plan and government program goals. You will collaborate closely with an innovative, multidisciplinary team to improve patient outcomes, enhance quality measures, and reduce avoidable costs. The ideal candidate is adaptable, tech-savvy, comfortable working independently in the field or remotely, and committed to patient-centered, evidence-based care in a rapidly evolving healthcare environment.
Required Qualifications
- Nurse Practitioner (NP) license/board certification
