Collaborate with a multidisciplinary team to improve patient outcomes, enhance quality measures, and reduce avoidable costs. Adaptable to working independently in the field or remotely, delivering patient-centered, evidence-based care in a rapidly evolving healthcare environment.
Required Qualifications
- Nurse Practitioner license
- Board-certified Nurse Practitioner
- Experience with in-home health assessments
- Ability to conduct clinical evaluations
- Experience with virtual/telehealth is a plus
