Job Summary
Conduct outpatient primary care for 15-17 patients per day across two integrated clinics in Grand Junction, Colorado, with no on-call rotation. Manage a broad scope of practice including wellness visits, chronic disease management, and in-office procedures, while collaborating with behavioral health providers, physicians, MAs, and RNs on weekly team-based pods. Deliver services to diverse populations including Medicaid, Medicare, and uninsured patients through a sliding fee discount scale. Work within a Federally Qualified Community Health Center that provides outreach to homeless and migrant farm workers.
Required Qualifications
- Master's degree, equivalent education, or higher degree from an accredited APN/NP program
- Colorado license to practice as an APN/NP
- DEA license to prescribe medication
- Life Support certification (BLS at minimum)
- Maintenance of Continuing Education as required by your APN/NP credentialing board and state medical board
Desired Qualifications
- Two years of related experience as an APN/NP
