Job Summary
Nurse Practitioner/Physician Assistant needed to provide clinical services for residents in Skilled Nursing & Rehabilitation facilities. Responsibilities include directing and coordinating nursing care, conducting physicals and annual assessments, completing acute visits, early identification and management of chronic conditions, timely admission histories and physicals, Medicare certifications, discharge planning and prescriptions, facilitating advanced care planning, on-call coverage, and documentation of assessments and plans of care in the EMR. Collaboration with physicians and staff, staff supervision and evaluation, ongoing policy review, and contributing to continuing education. Requires a current NP or PA license, DEA certificate, experience in adult/geriatric care, and ability to work in long-term care settings; on-call availability; strong communication and teamwork, and proficiency with Microsoft Word/Outlook.
Required Qualifications
- Current, unrestricted Nurse Practitioner license from the Board of Nursing/PA License from the Medical Board in the state of assignment or the ability to obtain
- DEA certificate current or the ability to obtain
- Clinical background in adult, family or geriatric specialties preferred
- Past experience working in long-term care, a nursing home, or with seniors in other settings preferred
- Strong communication and relationship building skills preferred
- Intermediate level of proficiency in PC based word processing, including Microsoft Word and Outlook
- Willing to be ‘on call’ for Facilities as needed
- Dependable, flexible, and able to work and cooperate well with all nursing personnel and other departments
- Ability to make decisions regarding nursing problems, work well under pressure, and problem solve
