Job Summary
Provide total care for patients in a Lake Placid primary care office in collaboration with the physician of record and patient care team. Responsibilities include performing patient assessments, creating care plans, diagnosing and treating patients, interpreting x-rays and lab results, writing prescriptions, taking histories, providing surgical assistance, ordering home health, therapies, and DME as indicated, and contributing to symptom management and advanced care planning for patients with chronic life-limiting illness. Maintain ongoing communication with the Primary Care Provider to ensure continuity and coordination of care, and consult with supervising physician on complicated diagnoses and treatment decisions; may facilitate end-of-life care with regional palliative/hospice services; complete documentation and billing per protocol. This role requires Florida licensure in good standing and works within a multidisciplinary team focused on evidence-based, patient-centered care.
Required Qualifications
- Licensed as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) in the State of Florida and in good standing
