Job Summary
Deliver comprehensive PM&R care to geriatric and post-acute patients within skilled nursing facilities in Charlotte, NC. Collaborate with interdisciplinary teams including nursing, therapy, and facility administration to develop individualized treatment and preventative care plans. Lead initiatives to reduce hospitalizations and improve functional outcomes while ensuring continuity of clinical excellence. Build strong relationships with patients, families, and staff to support patient-centered care. Requires an active North Carolina Nurse Practitioner License, DEA, and CDS. Full-time, day shift with flexible scheduling.
