Job Summary
Conduct comprehensive primary care visits and physical exams in the comfort of a patient's home. Create longitudinal care plans for high-risk patients to manage chronic disease and prevent ER visits by treating acute needs at home. Partner with an interdisciplinary team to ensure seamless, safe care transitions while addressing social determinants of health. Manage a dedicated group of patients with full support from the care team, exercising true autonomy in problem-solving. Requires an active, unencumbered NP license with Board Certification, a Master's degree, and a valid driver's license for local field-based travel. WellBe Senior Medical replaces profit-driven volume with Value-Based Care delivered directly in the home.
Required Qualifications
- Master's Degree (NP)
- Active, unencumbered NP license
- Board Certification (ANCC or AANP)
- Valid driver's license
- Vehicle for local field-based travel
- Legally authorized to work in the United States without the need for employer sponsorship now or in the future
- Successful completion of a pre-employment drug screening
- Compliance with the company's Drug-Free Workplace Policy
- Successful completion of a background check
- Satisfactory results of monthly FACIS (Fraud and Abuse Control Information System) checks
