Job Summary
Nurse Practitioner provides ongoing, comprehensive health care to clients within priority populations, including assessment, diagnosis, counselling, screening, referral, education, treatment and follow-up, within an interdisciplinary team and in alignment with the Expanded Nursing Services for Patients Act. The NP may work at any GCHC site or community locations, collaborates with physicians, and may share on-call duties. Requires registration in the Extended Class with the College of Nurses of Ontario and membership in the Nurse Practitioners Association of Ontario. Must uphold patient confidentiality and ethical standards, and may involve offsite/community work. Ability to address determinants of health, reduce barriers to care, and work with diverse, vulnerably positioned communities is essential.
Required Qualifications
- Registration in the Extended Class with the College of Nurses of Ontario
- Member of the Nurse Practitioners Association of Ontario
- Criminal Record Check and Vulnerable Sector Screen prior to start date
- Experience working with communities and individuals who face barriers to accessing health care
- Ability to speak multiple languages is an asset
- Proficient in the use of computers and various software including Word, Outlook, PSS and electronic health records
