Job Summary
Provide comprehensive primary care services to adult patients under physician collaboration, evaluating and managing acute and chronic conditions including diabetes, hypertension, obesity, and cardiovascular risk factors. Perform preventive care visits, annual wellness exams, and Medicare wellness visits while ordering, interpreting diagnostic tests, and prescribing medications per New York State regulations. Educate patients on disease management and lifestyle modification, document encounters in the electronic medical record, and coordinate care with specialists and ancillary services. Support quality initiatives and evidence-based care standards within a six-physician team in East Hills, NY. Requires NY licensure, national board certification, and BLS/ACLS/DEA.
Required Qualifications
- Licensed Nurse Practitioner in the State of New York (or eligible)
- National board certification
- BLS/ACLS/DEA
- Minimum 1–2 years of experience in internal medicine, primary care, or adult medicine preferred (new graduates with strong clinical rotations will be considered)
