Job Summary
Nurse Practitioner or Physician Assistant needed for Part-Time In-Home Health Evaluations with Signify Health (CVS Health). Conduct 45-minute in-home visits, including medication and medical history review, physical evaluation, optional diagnostic tests (e.g., spirometry, diabetic eye exams) per health plan. Utilize an iPad-based clinical workflow; not required to prescribe medicines or alter treatment. Travel locally within licensed states; also may conduct virtual visits. Primary workplace is members' homes; environments may include pets or allergens. Positions include benefits eligibility for part-time staff; anticipated 40 hours/week with competitive pay. Active, unrestricted licenses, board certification, and multi-state licensure are emphasized.
