Job Summary
In-Home Health Evaluations for Part-Time Nurse Practitioner or Physician Assistant: conduct at-home health visits (about 45 minutes) including medication and medical history review and physical evaluation; may include additional diagnostic tests if ordered by health plan; use an iPad and clinical workflow; no prescribing or altering treatment required. Travel locally within licensed states as needed; virtual visits as needed; primary workplace is member's home; environments may include pets and allergens. Responsibilities focus on engaging with members, bridging gaps in care, and delivering personalized health evaluations in the home setting. Qualifications include active/unrestricted licenses, board certification, and multi-state licenses (preferred).
