Job Summary
Provide in-home health evaluations for health plan members, performing physical evaluations, reviewing medical history, and coordinating care during 45-minute visits at the patient's home. Utilize an iPad and clinical workflow; visits may include optional diagnostic tests as ordered by the health plan. Roles require NP or PA with board certification and active licenses, willingness to travel locally within licensed states, and occasional virtual visits. Full-time role with 40 hours per week; benefits include health/dental/vision, PTO, CEU allowance, 401K, malpractice coverage. Travel to member homes is a core aspect of the role, with the primary workplace being members' residences.
