Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad and clinical workflow for each visit while connecting one-on-one with patients to answer health questions and build trusted relationships. Fulfill services including preventive care products such as bone density screening as ordered by the health plan. Perform virtual visits as needed and travel locally within licensed states. Work full-time hours with an anticipated weekly schedule of 40 hours.
