Job Summary
Provide in-home health evaluations as a Part-Time Nurse Practitioner or Physician Assistant, conducting 45-minute up-close visits in members' homes to review medical history, perform physical evaluations, and, if applicable, administer diagnostic tests (e.g., spirometry, bone density screening) per health plan orders. Work with Signify Health to deliver Diagnostic & Preventive Services, travel locally to multiple Colorado locations (San Miguel, Dolores, Montezuma, San Juan, La Plata, Hinsdale, Mineral, Archuleta, Rio Grande, Conejos, Denver) and perform virtual visits as needed using an iPad-enabled clinical workflow. Primary workplace is the member’s private residence; environments may include pets, secondhand smoke, and other allergens. Part-time role with benefits eligibility; board-certified NP/PA with active licenses and multi-state privileges; requires flexibility to travel within licensed states and to provide care with a patient-centered approach.

- This pay range represents the base pay for each In-Home Health Evaluation (and associated visit) completed by a physician.
