Job Summary
Conduct 45-minute in-home health evaluations involving medication reviews, physical exams, and diagnostic tests like spirometry and diabetic eye exams. Utilize an iPad-based clinical workflow to provide comprehensive preventive and diagnostic services while connecting one-on-one with patients. Travel locally and within licensed states as needed, and perform virtual visits when required. Work in diverse home environments that may include pets, secondhand smoke, or airborne allergens. This role supports Signify Health's mission to bridge care gaps by visiting millions of people nationwide to simplify health care.
