Job Summary
In-Home Health Evaluations by Part-Time Clinician (Nurse Practitioner or Physician Assistant) to bring personalized health assessments to members at their homes. Role involves one-on-one visits (~45 minutes each) including medication/history review, physical evaluation, optional diagnostic tests (e.g., spirometry, diabetic eye exams), using an iPad workflow, and no requirement to prescribe, order labs, or alter current treatment. Flexible to travel locally within licensed states; virtual visits as needed; primary duties include delivering Diagnostic & Preventive Services and connecting members with appropriate care in their homes.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Able to cover the following primary locations: Penobscot, ME
