Job Summary
Nurse Practitioner or Physician Assistant needed for Part-Time In-Home Visits with Signify Health (CVS Health). Conduct 45-minute in-home health evaluations in members' residences, including health history review, physical evaluation, and appropriate diagnostic tests as ordered by the health plan. Use an iPad and standard clinical workflow; no prescribing or altering current treatment. Primary duties include building trusted relationships, visiting members nationwide as part of a national clinician network, and traveling locally within licensed states as needed. Requires active, unrestricted licensure, board certification, and ability to cover Wilmar and surrounding locations; virtual visits may be conducted as needed. Part-time employees receive select benefits and malpractice coverage. Anticipated weekly hours: 40. Pay range noted as $41.85-$90.13 per hour; compensation varies by experience, education, geography, and other factors.
