Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical examinations, and diagnostic tests such as spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow to perform these visits while reviewing patient needs and providing education. Fulfill all Signify Health services including diagnostic and preventive offerings, conduct virtual visits as required, and travel locally within licensed states. Work full-time with 40 weekly hours in the Brainerd/Duluth/Minneapolis area, covering counties including Carlton, Lake, Saint Louis, Pine, Kanabec, Aitkin, and Isanti.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
