Nurse Practitioner, In-Home Visits

In-Home Health Evaluations by a Nurse Practitioner or Physician Assistant (Full-time). Conduct 45-minute visits in members' homes to review medication history, perform physical evaluations, and, when ordered by the health plan, administer diagnostic tests (e.g., spirometry, bone density screening). Use an iPad and standardized clinical workflow; no requirement to prescribe, order labs, or modify current treatment plans. Travel locally within licensed states as needed; some virtual visits may be conducted. Primary workplace is the member's private residence; environments may include pets or secondhand smoke.
