Part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations. Visits last about 45 minutes, include medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, eye exams). Use an iPad and clinical workflow; not required to prescribe medicines or order labs or alter current treatment. Engage with members at home to bridge gaps in care, traveling locally to member residences; primary workplace is the member's home. Roles emphasize compassionate, personalized care with flexible in-home evaluation services across a national clinician network.
