Job Summary
Conduct in-home primary care visits and comprehensive assessments for patients enrolled in the Chronic Care Management Program. Manage chronic conditions including diabetes, hypertension, and CHF through physical exams, medication management, and lab draws performed in the home. Develop and adjust care plans while delegating follow-up needs to the RN and Social Worker team. Educate patients and caregivers on treatment and prevention, then document visits accurately in the EMR. This role offers a 4 or 5-day schedule with no nights, weekends, or holidays, featuring a starting salary of $120,000+ and mileage reimbursement. You will practice in Burlington and Camden County, supported by dedicated care managers and optimized daily routes.
Required Qualifications
- Licensed Nurse Practitioner (NP) in the state of practice
- Strong experience in primary care and chronic disease management
- Comfortable working independently in a field-based role
- Ability to perform basic in-home procedures (including lab draws)
- Strong clinical judgment and patient communication skills
- Valid driver's license and reliable transportation
- Willingness to travel within service area
- CPR/BLS certification
