Nurse Practitioner, Immediate Primary Care

Develop and manage individualized care plans for patients with memory disorders, including Alzheimer's disease, mild cognitive impairment, and Lewy body dementia. Order and interpret diagnostic testing such as labs, imaging, and cognitive assessments while providing medication management and longitudinal follow-up. Deliver education and support to patients and caregivers within a multidisciplinary team that includes neurologists, neuropsychologists, and research staff. Participate in clinical research and innovative care models focused on dementia diagnosis and treatment.
