Job Summary

Conduct age-appropriate patient assessments including history taking and physical examinations consistent with department protocol and Kaiser Permanente policy. Perform ongoing systematic data collection focusing on physiologic, psychosocial, cultural, spiritual, and cognitive status while providing patient education and advocacy. Collaborate with supervising physicians and clinicians to ensure effective patient care, review medical records for diagnostic studies, and delegate duties to team members. Maintain communication with health care providers to promote continuity of care and participate in performance improvement and research activities. Practice under established procedures for nephrology conditions, including renal failure and dialysis, while demonstrating proficiency in computer documentation and emergency protocols.

Required Qualifications

  • Minimum of 6 months experience as a nurse practitioner
  • One (1) year (within the past three (3) years) experience as a NP providing care for kidney (nephrology) conditions, including renal failure who may require dialysis
  • Masters Degree in Nursing
  • Completion of Masters Program in Adult NP, Family Practice NP, Acute Care NP or Pediatric NP
  • Adult, Family or Acute NP if practice limited to adults
  • Family or Pedi NP if practice limited to pediatric patients
  • Acute Adult NP or Acute Pediatric NP if only caring for specific patient population of adult or pediatrics
  • Nurse Practitioner Certificate (California)
  • Registered Nurse License (California)
  • Nurse Practitioner Furnishing Number (California)
  • National Provider Identifier required at hire
  • Nurse Practitioner National Certificate
  • Basic Life Support
  • Must meet credentialing requirements established for nurse practitioners within the Kaiser system
  • Masters Degree in Nursing and National Certification from one of the following organizations: American Academy of Nurse Practitioners; American Credentialing Center; National Certification Corporation for Obstetric, Gynecologic and Neonatal Nursing Specialties; National Certification Board of Pediatric Nurse Practitioners and Nurses
  • Candidates must meet requirements to apply for PTAN (Provider Transaction Account Number) at the time of hire and apply within 30 days of hire
