Job Summary
Nurse Practitioner in Houston, TX for DaVita IKC, hybrid remote with travel across an assigned geographic area to dialysis clinics and nephrology practices. Responsibilities include completing Comprehensive Health Evaluations (CHEs) for CKD/ESKD patients, prescribing/medicating management in collaboration with nephrologists, leading clinical team coordination, and ensuring medical and psychosocial needs are addressed. Schedule is Monday–Friday, full-time with no nights/weekends/on-call, and flexibility to accommodate patient and nephrology partner availability. Requires 2+ years NP experience, active NP license, DEA eligibility within 90 days, CPR/BLS, valid driver’s license, and competency in telehealth, EHR/Cerner systems, and population-health concepts. Required Qualifications
- Nurse Practitioner license in state of practice
- DEA license or eligibility within 90 days
- Current CPR/BLS certification
- Valid driver’s license and insurance in the state
- Minimum 2 years’ NP experience
- Autonomous decision-making and clinical leadership skills
- Strong clinical judgment and ability to manage complex comorbidities
- Experience with telehealth/remote care settings
- MS Word, Excel, Outlook skills
- Ability to travel across assigned geographic area
