Job Summary
Nurse Practitioner (HouseCalls) to join Optum HouseCalls with regional travel across the Northeast. Conduct in-home annual health assessments for Medicare Advantage and other plan members in diverse home environments. Non-prescribing, field-based role focused on education, gap closure, and collaboration with primary care providers to improve health outcomes. Responsibilities include comprehensive in-home assessments (history, meds reconciliation, vitals, exams), screenings/POCT, documenting diagnoses, communicating findings to PCPs, recognizing urgent situations, patient education on disease processes and medications, and addressing social determinants of health with referrals. Requires active NP certification (ANCC/AANP), licensure, and a valid driver’s license with reliable transportation; 1+ years of clinical experience preferred; compensation range $109,500–$175,000 plus benefits and incentives. 85% travel across the Northeast; flexible scheduling (8am–8pm across 7 days), no on-call, no holidays. Locations include New York City, NY, US.
Required Qualifications
- For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations
- Active and unrestricted driver's license
- In addition to a driver's license, access to reliable transportation to complete home visits
- 1+ years of clinical experience (family, geriatric, or home health preferred)
Desired Qualifications
