Job Summary
Perform comprehensive health assessments, diagnose acute and chronic conditions, and develop individualized care plans for patients in their homes. Prescribe medications, order diagnostic tests, and provide patient education on health maintenance while coordinating care with physicians. Maintain accurate documentation of all encounters. This role requires an MSN, active New York license, and Mandarin bilingualism.
Required Qualifications
- Master of Science in Nursing (MSN) or higher from an accredited Nurse Practitioner program
- Active New York State Nurse Practitioner License
- Board Certified as a Nurse Practitioner
- Bilingual in Mandarin
- Proficiency in comprehensive patient assessments, diagnosis, treatment planning, and documentation
- Comfort with point-of-care testing and portable medical equipment
- Highly independent
- Excellent clinical judgment
- Strong communication skills
- Compassionate
- Adaptable to different home environments
- Minimum 3 days a week availability
