Job Summary
Conduct comprehensive health assessments, diagnose acute and chronic conditions, and develop individualized care plans for patients in their homes. Prescribe medications, order diagnostic tests, and provide patient education while coordinating care with physicians. Maintain accurate documentation of all encounters. Requires a Master of Science in Nursing, active New York State license, and bilingual Mandarin skills. Full-time or part-time roles (minimum 3 days/week) with a personal driver to the worksite.
Required Qualifications
- Master of Science in Nursing (MSN) or higher from an accredited Nurse Practitioner program
- Active New York State Nurse Practitioner License
- Board Certified as a Nurse Practitioner
- Bilingual in Mandarin
- Proficiency in comprehensive patient assessments, diagnosis, treatment planning, and documentation
- Comfort with point-of-care testing and portable medical equipment
- Highly independent
- Excellent clinical judgment
- Strong communication skills
- Compassionate
- Adaptable to different home environments
- Full-time or Part-time (minimum 3 days a week)
