Nurse Practitioner - House Calls (With a Driver)

Conduct comprehensive home visits for patients with chronic illnesses, mobility issues, or limited healthcare access, performing physical examinations, medication reconciliations, and diagnosing conditions. Develop individualized treatment plans, prescribe medications, and manage urgent medical issues while coordinating care with physicians and specialists. Provide patient and family education on health management, lifestyle modifications, and disease prevention. Maintain accurate documentation in electronic health records and schedule follow-up visits to monitor progress. Requires a Master's degree in Family or Adult Geriatrics NP, current New York State license, and National Board Certification. Bilingual Spanish speakers and those with EMR experience are encouraged to apply.
