Job Summary
Conduct initial and follow-up in-person visits at hospice sites to provide comprehensive medical care, symptom management, and palliative treatment for terminally ill patients. Perform history and physical exams, interpret diagnostic tests, formulate diagnoses, and document findings in electronic medical records. Develop individualized plans of care, prescribe treatments, and coordinate hospice services while ensuring compliance with Medicare, Medicaid, and state regulations. Collaborate with interdisciplinary teams and community providers to ensure coordinated care, providing education and support to patients, families, and caregivers. Participate in provider after-hours and weekend on-call panels and offer back-up coverage for home-based palliative care patients. Requires Master's degree in nursing, one year of hospice experience, CA driver's license, and current licensure.
Required Qualifications
- Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program
- Current nursing licensure in State
- CPR certification
- Master's degree in nursing
- minimum of one (1) year Nurse Practitioner experience in palliative care and/or hospice care
- geriatrics, oncology or equivalent
- Accreditation from an approved certifying body for advanced practice nursing
- Certification in Hospice and Palliative Care
- Advanced knowledge of end-of-life care
- Advanced knowledge of general inpatient hospice (GIP) symptom management
- Advanced knowledge of hospice eligibility criteria
- Excellent observation skills
- Excellent verbal and written communication skills

Desired Qualifications
