Job Summary
Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans. Prescribe medications, complete hospice recertification face-to-face assessments, and manage symptom control through collaboration with interdisciplinary teams. Educate patients and families on end-of-life medical, emotional, and spiritual needs while maintaining accurate documentation and adhering to HIPAA standards. Requires current NP or APRN license, accredited degree, minimum two years of RN experience, and proficiency in electronic health records. Hourly base range is $35.00 for orientation and meetings, with face-to-face visits paid at $100.00. Valid driver's license, reliable transportation, and ability to pass background and drug screening are required.
Required Qualifications
- Hold a current Nurse Practitioner (NP) or Advanced Practice Registered Nurse (APRN) license in the state(s) of practice.
- Degree from an accredited NP program
- Minimum two years of RN experience
- Valid driver's license
- Reliable transportation
- Automobile insurance
- Ability to pass background and drug screening
