Job Summary
Hospice Nurse Practitioner role in Edison, NJ providing advanced clinical care to patients in hospice; initiate communication with attending physicians and hospice staff, monitor patient progress, coordinate discharge planning, and collaborate with the care team. Responsible for prescribing medications as permitted by state regulations, performing face-to-face assessments (including home visits when possible), and ensuring continuity of care with the care team and case manager. Requires Masters degree in nursing, an unencumbered APN license in practicing states, CPR (preferred), and ability to travel to all business locations.
Required Qualifications
- Masters degree in nursing
- Current and unencumbered advanced practice nurse (APN) license in the state(s) practicing
- One year of experience as an NP
- Board-certified by a nationally recognized advanced nurse practice organization
- CPR certification (preferred)
- Licensed driver who can travel to all business locations
- Meet the regulations and requirements of the state(s) in which the program provides services
