Job Summary
Nurse Practitioner provides direct patient care at a specific Hosparus Health community location, serving as hospice attending practitioner with billable visits and non-billable recertification visits, and rotating to cover weekday/weekend call. Collaborates with the interdisciplinary group (IDG) and participates in education, outreach, and leadership activities for the community location. Role includes potential additional patient-care or administrative duties at the discretion of the Senior Nurse Practitioner or VP/Chief Medical Officer. Schedule is Monday–Friday 8:30am–5:00pm with occasional on-call. Location is Louisville, KY.
Required Qualifications
- MSN or higher-level Adult Nurse Practitioner required
- Licensure (or eligibility for licensure) for advanced nursing practice in KY and IN, including DEA and controlled substance permits
- Minimum of one year experience in full time advanced nursing practice required
- Primary care, palliative care, hospital-based, or hospice experience preferred
- Fulfill requirements of on-call duty policies
- Valid driver’s license per policy
- Use of technology-based tools, including electronic medical record, strongly preferred
