Job Summary
Nurse Practitioner opportunity for in-home, Medicare-focused care within P3 Health Partners’ Home Program in the Tucson, AZ area. Responsibilities include in-home health risk assessments, comprehensive physical examinations, diagnosing and treating conditions, ordering and interpreting tests, coordinating referrals, educating members on chronic conditions and preventive care, and participating in a rotating on-call pool (including after-hours, weekends, and holidays). The role involves collaborating with a multidisciplinary team to support patients in a home-visit model and may require travel up to 100 miles for visits. Qualifications include a Medical/Master’s degree, an active NP license (or ability to obtain by start date), an unrestricted State DEA license, a minimum of 3 years of nursing experience, a valid driver’s license and automobile insurance, and comfort with home-based assessments. Required Qualifications
- Medical/Master’s Degree and Certification
- Active NP License or ability to obtain by start date
- Unencumbered licensure (no probation or restrictions)
- Unrestricted State DEA license
- Minimum of 3 years of nursing experience
- Valid State Driver’s License with clean DMV record
- Vehicle Insurance copy
- Ability to lift 30 pounds, navigate stairs and perform physical assessment
- Reliable transportation to travel up to 100 miles for home visit assessments
- CME hours and reimbursement preferred for geriatrics
- Home Health care or home visit experience preferred
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care (or ability to obtain)
- ICU or ER experience
- Technically savvy
