Nurse Practitioner-Hematology/Oncology

Conduct longitudinal survivorship care for women with breast or gynecological cancers transitioning from active treatment to long-term follow-up. Develop and expand a comprehensive survivorship care model to improve continuity, reduce symptom burden, and coordinate care across specialties. Manage long-term treatment effects and support patients following completion of active oncology therapy. Requires 2+ years' experience, an AZ license, DEA, and oncology certification desired. Full-time 1.0 FTE with four 10-hour shifts and four to five Saturdays annually.
