Job Summary
Outpatient heart failure NP/PA role focusing on ongoing management and follow-up care. Conduct comprehensive patient assessments, order and interpret diagnostic studies, develop and implement treatment plans in collaboration with supervising physicians, provide medication management and patient education on disease management and lifestyle modifications, and coordinate care with primary care providers and specialists. Collaborates with heart failure cardiologists and a multidisciplinary team, supports clinic workflow, and documents encounters and care plans in the electronic health record. Comprehensive training in heart failure management and ventricular assist devices (VADs) is provided, with opportunities to develop specialized expertise in heart failure and advanced therapies within a collaborative, team-based outpatient setting.
Required Qualifications
- Current license and active registration as a Nurse Practitioner or Physician Assistant in New York State
- Master's degree from an accredited Nurse Practitioner program or completion of an accredited Physician Assistant program
- Interest in or experience with cardiovascular or heart failure care preferred
- Familiarity with electronic medical records; Epic experience preferred
- Strong communication and collaboration skills
- Commitment to patient-centered care and maintaining HIPAA confidentiality standards
