Job Summary

Provide advanced medical care and clinical support to hospice patients and their families by evaluating, diagnosing, and managing pain, symptoms, and terminal illnesses. Conduct patient visits in private residences, skilled nursing facilities, assisted living communities, hospitals, and other care settings, including telehealth when appropriate. Collaborate with attending physicians and the interdisciplinary team to develop individualized plans of care, while communicating regularly with referral sources regarding new patient referrals. Complete required face-to-face encounter documentation and maintain accurate clinical records in compliance with regulations. Provide education and clinical guidance to patients, families, caregivers, and staff regarding disease progression and symptom management.

Required Qualifications

  • Graduate of an accredited School of Nursing and accredited Nurse Practitioner program
  • Current, active Nurse Practitioner license in the state of Rhode Island
  • Current DEA license
  • Current BLS/CPR certification
  • Master's degree
  • Minimum of one (1) year of Nurse Practitioner experience
  • Minimum of one (1) year of hospice or palliative care experience
  • Current certification from an approved advanced practice nursing certifying body
  • Valid driver's license with reliable and insured transportation
  • Excellent clinical assessment, communication, critical thinking, and collaboration skills
