Job Summary
Travel daily to assigned nursing homes and assisted living facilities to address dementia, Alzheimer's disease, and other neurocognitive disorders. Provide initial evaluations, continuing therapy services, and cognitive testing while documenting the need and extent of services for each patient. Create and maintain positive professional relationships with client facilities, patients, and their families within a multidisciplinary team focused on patient safety, functionality, and dignity. Enjoy the autonomy of private practice with an assigned caseload, supported by corporate clinical leadership and in-house scheduling, coding, and billing departments.
