Job Summary
Nurse Practitioner provides initial evaluations, continuing therapy services, and cognitive testing for aging patients with dementia and neurocognitive disorders in nursing homes and assisted living facilities. The role involves documenting needs and service extent for each patient, maintaining professional relationships with facilities, patients, and families, traveling daily from home to assigned facilities, and collaborating within a multidisciplinary clinical leadership system with administrative support for scheduling, coding, and billing. The position requires NP licensure, autonomous function with regional manager support, strong time management, and adherence to pre-employment checks and credentialing.
Required Qualifications
- Proof of Nurse Practitioner licensure
