Job Summary
Travel daily to assigned facilities to provide initial evaluations, continuing therapy services, and cognitive testing for patients with dementia and Alzheimer's disease. Document the need and extent of services for each patient while creating positive professional relationships with client facilities, patients, and their families. Work within a multidisciplinary team to promote patient safety, functionality, and dignity, supported by corporate clinical leadership and administrative departments for scheduling, coding, and billing.
