Job Summary
Travel daily to assigned nursing homes and memory care facilities to provide initial evaluations, continuing therapy services, and cognitive testing for patients with dementia and neurocognitive disorders. Document the need and extent of services for each patient while creating and maintaining positive professional relationships with client facilities, patients, and their families. Function autonomously with regional Area Manager support as part of a multidisciplinary team focused on enhancing patient safety, functionality, and dignity.
Required Qualifications
- Proof of Nurse Practitioner licensure
- Ability to work as part of an interdisciplinary treatment team
- Good time management skills
- Able to function autonomously with regional Area Manager support
- Pass pre-employment background check and drug screening
- Complete credentialing process
