Job Summary
Travel daily to assigned nursing homes and assisted living facilities to provide initial evaluations, continuing therapy services, and cognitive testing for patients with dementia and neurocognitive disorders. Document the need and extent of services for each patient while creating and maintaining positive professional relationships with client facilities, patients, and their families. Work autonomously within a multidisciplinary team to promote patient safety, functionality, and dignity, supported by regional Area Manager oversight and in-house administrative departments for scheduling, coding, and billing.
Required Qualifications
- Proof of Nurse Practitioner licensure
- Ability to work as part of an interdisciplinary treatment team
- Good time management skills
- Able to function autonomously with regional Area Manager support
- Pass pre-employment background check and drug screening
- Complete credentialing process
