Job Summary
Provide comprehensive care to patients in nursing home settings by conducting assessments, diagnosing conditions, and developing treatment plans. Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care, prescribe medications and therapies as needed, and educate patients and families on health management and preventive care. Maintain accurate and up-to-date patient records while adhering to all regulatory and organizational policies and procedures. Requires an MSN, active New York State license, board certification, and at least six months of experience. Full-time or part-time roles available in Brooklyn, NY.
Required Qualifications
- Master's degree in Nursing (MSN) from an accredited program
- Active New York State Nurse Practitioner license and Board Certification
- At least 6 months of experience as an NP
- Proficiency in eClinicalWorks EMR
- Strong communication and problem-solving skills
- Ability to work independently and as part of a team
- Full-time or Part-time employment (Minimum of 3 days a week)
