Nurse Practitioner- Geriatric/Adult Clinic- Dublin, Ohio

Provide outpatient care to geriatric patients in a Dublin, Ohio clinic, managing high-acuity transitions from acute hospital care to rehabilitation. Collaborate on patient care with physicians, therapy, nursing, and social work while ordering and interpreting diagnostic tests, managing medication reconciliation, and performing minor procedures. Perform comprehensive evaluations, including history and physicals and progress notes, and document all encounters accurately in the electronic medical record. Act as the primary clinical liaison for families to align medical interventions with goals of care, partnering with the Medical Director to ensure documentation meets CMS Five-Star Quality Rating requirements. Manage a daily census of 12–15 encounters with shared telephonic on-call rotation to support 24/7 clinical stability.
