Job Summary
The Nurse Practitioner at Senior Community Care of Michigan PACE provides primary care to participants under the supervision of the Medical Director. Responsibilities include performing physical assessments, developing care plans, and providing education to participants and caregivers. The role involves evaluating physical complaints, coordinating care with Primary Care Physicians, and conducting semi-annual reassessments. Candidates must hold a valid R.N. license, have graduated from a professional nursing program, and be certified as a nurse practitioner. A minimum of one year's experience with the elderly is required.
Required Qualifications
- Licensed as Registered Nurse (R.N.) in the state of employment
- Graduate of a school of professional nursing required
- Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing
- A minimum of one year’s experience in working with the frail and elderly population required
- Shall have either training or related experience in the job assigned
Desired Qualifications
- Certified as a Geriatric Nurse Practitioner (G.N.P.) with prescriptive authority preferred
- Two (2) years’ experience as a nurse practitioner in a geriatric setting desirable
