Job Summary
Nurse Practitioner (Full-Time) under Medical Director supervision provides primary care to Senior Community Care of Michigan PACE enrollees. Performs comprehensive H&Ps for new participants and semi-annual reassessments, develops and implements plans of care with the Interdisciplinary Team, educates participants and caregivers, and coordinates with PCPs and Medical Directors within VOANS Skilled Nursing Facilities. Requires in-state RN licensure, NP certification, nursing degree, and geriatric experience; includes in-person care, travel to participant homes, and rotating after-hours on-call with occasional weekends. Schedule is Monday–Friday, 8:00 AM–4:30 PM, in Lansing, MI with local travel.
Required Qualifications
- Licensed as Registered Nurse (R.N.) in the state of employment
- Graduate of a school of professional nursing required
- Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing
- A minimum of one year’s experience in working with the frail and elderly population required
- Shall have either training or related experience in the job assigned
