Job Summary
Nurse Practitioner position (Full-Time) in Lansing, MI with Senior Community Care of Michigan PACE. Provide primary care under Medical Director supervision to frail elderly participants, perform comprehensive histories and physicals, develop care plans with the Interdisciplinary Team, conduct semiannual reassessments, manage in-person resident care at VOANS Skilled Nursing Facilities, coordinate with Primary Care Physicians, participate in discharge planning, and deliver patient and caregiver education. On-site schedule is Monday–Friday 8:00 AM–4:30 PM with rotating after-hours on-call and occasional local travel to participant homes. Preference for Geriatric Nurse Practitioner with prescriptive authority; two years of geriatric NP experience desirable.
Required Qualifications
- Licensed as Registered Nurse (R.N.) in the state of employment
- Graduate of a school of professional nursing required
- Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing
- A minimum of one year’s experience in working with the frail and elderly population required
- Shall have either training or related experience in the job assigned
