Provides comprehensive primary care to residents in Ontario long-term care homes, including conducting admission health assessments, coordinating with physicians and the interdisciplinary team, leading quality improvement initiatives, and delivering formal and informal teaching and coaching in clinical care. Adheres to the Resident Bill of Rights and relevant provincial legislation (Fixing Long-Term Care Homes Act), participates in resident and family care conferences, and offers outreach and transitional services for residents returning to the community; supports pharmacological, counselling, and supportive interventions and collaborates to manage resident plans of care.
