Job Summary
Collaborate with physicians in an active practice to deliver end-of-life and palliative care while serving as a consultant for area hospice programs. Advocate for hospice initiatives and provide education to healthcare professionals, acting as a bridge between hospitals and hospices to improve care for terminally ill patients. Manage Medicare patients with a minimum of two years' experience and apply advanced clinical skills within the Henry County/SW Atlanta, Clayton, Cobb, and Cherokee territories. Requires a Georgia Advanced Practice Nursing license, gerontology or family nurse practitioner certification, and CPR certification.
Required Qualifications
- Graduate from an accredited school of nursing
- Master's degree in Nursing
- Reliable transportation
- GA driver's license
- Valid vehicle insurance
- Current Georgia license for Advanced Practice Nursing
- Certification as either Gerontologic Nurse Practitioner by the American Academy of Nurse Practitioners Certification Program or the American Nurses Credentialing Center, or Family Nurse Practitioner or Adult Nurse Practitioner by the American Academy of Nurse Practitioners Certification Board or the American Nurses Credentialing Center
- CPR Certified
- Minimum of two years in the management of Medicare patients
- Knowledge of Medicare/Medicaid and other funding source regulations as applied to home health care services
- Demonstrates knowledge and competence in advanced practice clinical skills, including the nursing process and utilization management
