Job Summary
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, focusing on overall health, medication reviews, and physical assessments. Perform pre-scheduled visits with health plan members, utilizing provided patient-specific forms to identify care barriers. Complete an average of 6-9 visits daily with rates up to $135% via market multipliers and pay-per-test bonuses. Work flexible Monday-Sunday shifts as an independent contractor with no night or weekend requirements. Receive malpractice coverage, travel support including airfare and hotel, and availability incentives.
Required Qualifications

- State License Required
- Independent Contractor, 1099 status
