Job Summary
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, focusing on overall health, medication reviews, and physical assessments. Identify barriers to care and educate patients on treatment plans during pre-scheduled visits. Perform assessments with health plan members in their homes, utilizing provided patient-specific forms without prescribing medicine or altering existing regimens. Earn up to $4,000 weekly based on an average of 6-9 daily visits, with additional pay-per-test incentives and market multipliers. Work flexible shifts Monday through Sunday as an independent contractor with malpractice coverage.
